How it is calculated
The score is a plain sum of points: score = CHF + hypertension + diabetes + vascular disease + stroke/TIA + age points + sex. Heart failure, hypertension, diabetes and vascular disease are entered as 0 (absent) or 1 (present). Prior stroke or TIA is entered as 0 or 2. Age points are 0 (under 65), 1 (65 to 74) or 2 (75 and over). Female sex adds 1 point and male adds 0. Any other value, such as 2 for hypertension or 1 for stroke, returns no result. Fields left blank count as 0.
The maximum is 9. The score shown counts every point, but the tool labels the points without the female-sex point (as in the 2020 ESC guideline): 0 is Low risk, 1 is Moderate risk, and 2 or more is High risk. So a woman whose only point is her sex (score 1) reads Low risk, a woman with one other factor (score 2) reads Moderate, and a man with one factor (score 1) reads Moderate. It is an educational score, not a treatment decision.
Worked example
Take a 72-year-old woman whose only other condition is high blood pressure. Heart failure 0, hypertension 1, diabetes 0, vascular disease 0, stroke/TIA 0. Age 72 falls in the 65 to 74 band, so age points are 1. Female sex adds 1. Total: 0 + 1 + 0 + 0 + 0 + 1 + 1 = 3, which the tool labels High risk, because the label reads the points without the sex point (here 2). The same woman without hypertension would score 2, but with only 1 point besides sex she is labelled Moderate risk.
Common mistakes
- Entering 1 for a previous stroke or TIA: that factor is worth 2 points, so only 0 or 2 is accepted and 1 returns no result.
- Counting age twice: age points are one field with 0, 1 or 2, because the 65 to 74 and 75 and over bands are alternatives, not additive.
- Forgetting that sex is scored but not used for the label: a woman with no other factor has a score of 1 yet reads Low risk, because the label ignores the sex point, while a man with the same history scores 0 and also reads Low risk.
- Treating the labels as a diagnosis: the tool only maps the points without the sex point to Low, Moderate or High, and does not include bleeding risk or other clinical factors.
- Leaving an applicable field at its default: blank fields count as 0, so a condition you forget to tick lowers the total.

About this calculator
The CHA₂DS₂-VASc score adds up points for Congestive heart failure (1), Hypertension (1), Age ≥75 (2), Diabetes (1), prior Stroke/TIA/thromboembolism (2), Vascular disease (1), Age 65-74 (1), and Sex category female (1), producing a 0-9 score that stratifies annual stroke risk in patients with non-valvular atrial fibrillation.
Cardiologists, neurologists and primary care physicians use the score, endorsed by major cardiology guidelines (ESC, ACC/AHA), to decide whether a patient's stroke risk is high enough to justify long-term oral anticoagulation (warfarin or a DOAC) versus no antithrombotic therapy, weighing it alongside a separate bleeding-risk score like HAS-BLED.

Sex is scored, but the label ignores that point: a woman whose only point is her sex shows a score of 1 yet reads Low risk, while a woman with one other factor (score 2) and a man with one factor (score 1) both read Moderate. This is an educational tally, with fixed values of 0 or 1 (and 2 for prior stroke or TIA). Are you using it for study, or to follow a family member's care?
