Reticulocyte Production Index Calculator
Gauge bone-marrow response with the corrected reticulocyte production index.
Result
How it is calculated
Two steps, both using percentages. First the raw reticulocyte percentage is corrected for how low the hematocrit is: corrected retic % = retic % × Hct ÷ 45, using 45 as the normal hematocrit. Then it is divided by a maturation factor chosen from the hematocrit: 1.0 at 40 or above, 1.5 from 30 up to 40, 2.0 from 20 up to 30 and 2.5 below 20, the bands of the MSD Manual RPI calculator (after Hutchinson and Davey, Clinical Diagnosis and Management by Laboratory Methods). RPI = corrected retic % ÷ maturation factor. All three values are shown with two decimals. A reticulocyte value of 0 is allowed; it must not exceed 100. Hematocrit must be a percentage from 5 to 100. Values outside those limits show no result. The tool gives the index only and no interpretation, and laboratories may use a different normal hematocrit. Educational use only.
Worked example
Reticulocytes 6% and hematocrit 27%. Corrected retic = 6 × 27 ÷ 45 = 3.60 %. A hematocrit of 27 is in the 20 to 29.9 band, so the maturation factor is 2.00. RPI = 3.60 ÷ 2.00 = 1.80. Without the hematocrit correction the raw 6% would look like a strong marrow response, but the index shows a much more modest figure.
Common mistakes
- Entering the reticulocyte count as an absolute number (such as 90,000 per microlitre) instead of a percentage of red cells. Anything above 100 gives no result.
- Entering hematocrit as a fraction (0.27) instead of a percent (27). The tool rejects any hematocrit below 5 and shows no result, so type 27 and not 0.27.
- Entering hemoglobin where hematocrit is asked. The maturation factor bands are set by hematocrit, so hemoglobin values will land in the wrong band.
- Expecting a smooth maturation factor. It changes in steps at 20, 30 and 40, so a hematocrit of 29.9 gives 1.99 while 30 gives 2.67 for the same 6% reticulocytes.
- Entering impossible percentages. A reticulocyte value of 150, a hematocrit of 270 or a hematocrit below 5 is rejected and no index is shown.

About this calculator
The reticulocyte production index (RPI) corrects a raw reticulocyte percentage for two things that otherwise distort it: the degree of anemia and the premature release of immature reticulocytes into the bloodstream. The first step multiplies the reticulocyte percentage by the patient's hematocrit divided by a normal hematocrit (typically 45), giving the corrected reticulocyte count; the second step divides that figure by a maturation factor — here, following the MSD Manual's RPI calculator, 1.0 at a hematocrit of 40 or higher, 1.5 at 30 to 39.9, 2.0 at 20 to 29.9 and 2.5 below 20 — because “shift” reticulocytes released early from the bone marrow circulate and mature over more than the normal one day.
Hematologists use the RPI to work out whether bone marrow is compensating adequately for anemia. An RPI below 2 generally points to a hypoproliferative or maturation-defect process — iron deficiency, marrow suppression, or chronic disease — where the marrow is under-responding, while an RPI above 3 suggests the marrow is responding appropriately to increased red cell loss or destruction, as in hemolysis or acute blood loss.
This calculator applies both correction steps — hematocrit correction and maturation-factor adjustment — to a raw reticulocyte percentage and hematocrit value to return the RPI.
