Pack-Years Calculator
Calculate your smoking pack-years in one step
Result
How it is calculated
The tool computes pack-years = (cigarettes per day / 20) × years smoked. The divisor 20 is the number of cigarettes in one pack, so the first part converts your daily count into packs per day. Multiplying by the number of years gives the cumulative exposure, shown with two decimals. Neither input may be negative, and decimals such as 1.5 years or 7.5 cigarettes are accepted.
The calculation assumes a single steady habit over the whole period. If the amount changed, work out each stretch separately (for example 10 a day for 5 years, then 20 a day for 8 years) and add the pack-years together. The tool does not subtract years spent not smoking, and it does not cover pipes, cigars or vaping, whose pack equivalents are not defined here.
Worked example
Someone smoked 15 cigarettes a day for 12 years. In packs per day: 15 / 20 = 0.75. Multiply by the years: 0.75 × 12 = 9.00 pack-years. If the same person had smoked a full pack of 20 a day for the same 12 years, the figure would be 12.00.
Common mistakes
- Counting the years since starting instead of the years actually smoked; quit periods must be left out.
- Entering packs per day in the cigarettes field, such as 1 for a pack; the field takes single cigarettes, so a pack a day is 20.
- Using one average for a habit that changed a lot over time, which can hide a heavy period; split it into stretches and add the results.
- Assuming a pack always holds 20: in places where packs hold a different number, convert to cigarettes first, because the tool divides by 20.
- Treating pack-years as a measure of risk by itself; the same number can come from heavy short-term or light long-term smoking, which are not equal in effect.

About this calculator
A pack-year equals one pack (20 cigarettes) smoked per day for one year: pack-years = (cigarettes per day ÷ 20) × years smoked. Someone who smoked a pack a day for 20 years has the same 20 pack-year exposure as someone who smoked two packs a day for 10 years, even though their daily habits look very different.
Pulmonologists and primary care doctors use pack-years to quantify cumulative tobacco exposure and estimate lung cancer and COPD risk. It's also the exact threshold in clinical guidelines — the USPSTF recommends annual low-dose CT lung cancer screening for current or former smokers with at least a 20 pack-year history who are within 15 years of quitting.

Some lung-cancer screening guidelines use a pack-year threshold together with an age range and years since quitting, but the exact numbers differ between countries and change over time. This tool only produces the figure, so take it to a doctor, who can say whether screening applies to you. If packs where you live hold other than 20 cigarettes, tell us the size you see.
